This is probably not a bug to do with your XBOX...

For the mold of the wound, make sure you are using the microsil [the one that looks like toothpaste] on the lower wound. Get as close as possible- it might be easier to get it when the body is in the morgue.

For the bullet- get as close as possible to it a la pop up window.
